Biography Submission
Submit a short biography which will be sent out internally only as an informal hospital wide introduction on your behalf. This can include anything from where you were born, what schools you have attended, or your personal interests and hobbies. This can also be submitted later by emailing the information to HouseOfficerSubmissions@vetmed.ufl.edu, but should be completed during orientation at the latest.

Hosptial Security Badge/Gator 1 Card*

Hospital Security Badge/Gator 1 ID Card - Need New*
Upload a photo using the link below which will be utilized to have your dual hospital security badge/Gator 1 card created before your arrival. Photo requirements (similar to a passport photo): taken within the last 3 months and reflects your current appearance, background should be plain or off-white, full-face view, and you should be directly facing the camera.

Hospital Security Badge/Gator 1 ID Card - Already Have*
A badge modification request form will be submitted on your behalf so your accesses are updated in accordance with your new role/title. If you wish to have the title updated on your badge/card as well, Security will be notified based on your below selection. To have your title updated, please visit Security the week before or during your scheduled Orientation. Security is available Monday through Friday 8:00 a.m. - 3:00 p.m. for badge updates or at (352) 294-4444 if you need anything further.
Lab Coats
Please be sure of the size you are requesting as your name will be embroidered on them (i.e. Dr. Jane Smith). For questions or concerns regarding lab coats, please email HouseOfficerSubmissions@vetmed.ufl.edu and your inquiry will be forwarded to your specific coordinating contact. Women’s lab coats are Cherokee style #2410 with sizes running from XS to 3XL. Sizing information is available on the following website: http://www.cherokeeuniforms.com/product/10009/2410-WHT/36-lab-coat. Unisex lab coats are Cherokee style #1346 with sizes running from XS to 5XL. Sizing information is available on the following website: http://www.cherokeeuniforms.com/product/10009/1346-WHT/40-unisex-lab-coat. A total of two lab coats will be ordered except for Anesthesia, Diagnostic Imaging, Integrative Medicine, and Emergency & Critical Care. Only one will be ordered for these services.

Operating Room/Surgical Scrubs
Any O.R./surgical scrubs provided stay with the operating room service. They are unisex style in jade green. Once on-campus please refer all inquires on obtaining your O.R./surgical scrubs to Brandy Smith (O.R. & Anesthesia Manager).

Treatment Scrubs
Please be sure of the size you are requesting as your name will be embroidered on them (i.e. Dr. Jane Smith). For questions or concerns regarding treatment scrubs, please email HouseOfficerSubmissions@vetmed.ufl.edu and your inquiry will be forwarded to your specific coordinating contact. Any Cherokee brand scrub is available, but popular options include: - Top style #s: Unisex 4700 or 4777; Missy Fit 4801 or 4728 - Pant style #s: Unisex 4100 or 4001; Missy Fit 4044, 4101, or 4020 Other styles and sizing information is available on the following website: http://www.cherokeeuniforms.com If not confident about your style/size selection from just the website, a recommendation would be to visit a local scrub shop to try on options and then supply your details on this form. Once embroidered, scrubs are unable to be returned/exchanged and count towards your uniform allowance. A total of 4-5 scrubs sets will be ordered depending on overall pricing, except for Integrative Medicine as only 1 set will be ordered. All scrubs will be ordered in navy blue.
