Hospital Security Badge/Gator 1 ID Card – Need New* Upload a photo using the link below which will be utilized to have your dual hospital security badge/Gator 1 card created before your arrival.
Photo requirements (similar to a passport photo): taken within the last 3 months and reflects your current appearance, background should be plain or off-white, full-face view, and you should be directly facing the camera.

A total of two lab coats will be ordered except for Anesthesia, Diagnostic Imaging, Integrative Medicine, and Emergency & Critical Care. Only one will be ordered for these services. If you are a current House Officer on clinics (e.g. Intern transitioning to a Resident role) and wish to defer pre-ordering items please select the “Do Not Order” option – uniform ordering for your new role will be handled during/after Orientation instead.
Operating Room/Surgical Scrubs – Aquatic Animal Health Please be sure of the size you are requesting as your name will be embroidered on them (i.e. Dr. Jane Smith). Any O.R./surgical scrubs provided stay with the operating room service. They can be in any style (Guidelines: tops must not have waist level pockets; jogger-style pants are preferred), however must be jade green or surgical green. Please provide a link to the scrub style you wish to use. One (1) set of OR scrubs will be ordered.

If not confident about your style/size selection from just the website, a recommendation would be to visit a local scrub shop to try on options and then supply your details on this form. All scrub items are unable to be returned/exchanged and count towards your uniform allowance.
